BUSINESS PARTNERS - Vendor Certification

All new Contractors, Suppliers, and Vendors are required to complete and return the forms listed below and attach the requested documentation before your company may be considered an approved vendor.  Payments for services cannot be made for goods and services without the proper documents on file.

  • W-9 - Request for Taxpayer Identification Number and Certification (available online, click here.)

  • Certificates of Insurance:

    • Certificate of Liability Insurance from your insurance agent with a minimum of $1M in Liability Coverage.  This certificate must list 4 Seasons Management Group, LLC as an additional insured.

    • Certificate of Automobile Insurance from your insurance agent with a minimum of $1M in Liability Coverage.  This certificate must list 4 Seasons Management Group, LLC as an additional insured.

    • Certificate of Workers' Compensation Insurance (or in the case of a Sole Proprietor, the Sole Proprietor/Independent Contractor Workers' Compensation Indemnity Agreement must be completed and notarized - available online, click here.)

  • If you would like to be listed on our website as a Business Partner, please send a letter authorizing us to list your company including the proper URL for listing.  This letter must be sent on company letterhead (available online, click here.)

You will receive a response to your application within 30 days of your submission.
